Extract from the Court of Enquiry Summary

 

 

" A/C seen to dive into the ground on fire after an explosion was heard. C of I consider that explosion of photo flash in fuselage which blew the tail unit off and set the A/C on fire, was primary cause. No proof of reason for explosion of the photo flash. Rec that 28B type fuse be employed in place of the more dangerous P6. O/C concur. In future only flash will be carried and this will be positioned in the flare shute. ADC no definite evidence, concur C of I but clasify accident "Causes not fully established"
